How to Add the Microsoft Forms Add-In via Outlook on the Web
This guide walks through the process of installing the Microsoft Forms add-in for PowerPoint by joining the required Microsoft 365 distribution group. Following these steps ensures the add-in is properly installed and available within your Office applications.
Access Outlook on the Web
Log In
Open a web browser and navigate to: https://outlook.office.com
Sign in using your Microsoft 365 institutional credentials.
Navigate to Distribution Groups
Open Settings
Click the Settings (gear) icon located in the upper-right corner of the Outlook window.

Access Distribution Group Settings
Within the Settings panel:
• Select General
• Click Distribution groups
• Select the available portal link to manage group membership

You will be redirected to the Microsoft 365 Groups management page.
Join the Microsoft Forms Add-In Group
Access Groups
From the left-hand navigation menu:
• Select Groups

In the Groups tab, click the Join button.

Search for the Add-In Group
In the search field:
• Type osuo365-addin-forms
• Locate OSUO365-AddIn-Forms in the search results

Join the Group
• Select the group
• Click Join

Once joined, the system will automatically provision the add-in.
Installation Processing
Automatic Add-In Deployment
After joining the group, the Microsoft Forms add-in will be automatically installed in your Office applications, including PowerPoint.
Important:
This process may take up to a couple of hours to complete due to Microsoft 365 synchronization.
Verify Installation in PowerPoint
Once processing time has passed:

Open PowerPoint

Select My Add-ins

Confirm that Microsoft Forms in PowerPoint appears in your available add-ins

Troubleshooting
If the add-in does not appear after several hours:
• Confirm you successfully joined OSUO365-AddIn-Forms
• Sign out and back into PowerPoint
• Restart your computer
• Contact IT support if the issue persists
