Accessing Qualtrics

Starting March 3. 2025, Ohio State will implement a new process for requesting access to Qualtrics. Any college faculty member, staff member, or student needing a new Qualtrics account should visit the OTDI Self-Service portal to complete an access request form. Requestors will be asked to verify that they have completed the current fiscal year’s Institutional Data Policy or HIPAA and Institutional Data Compliance training. This training is mandatory before access can be elevated, as Qualtrics has access to S4 data. More information is available on the university’s Qualtrics blog.

A Reminder About Data Security

Please note that Qualtrics is not approved for the collection or storage of Restricted Data (S4) per Ohio State’ Institutional Data Policy. Examples of restricted data included Protected Health Information (PHI), social security numbers, credit card numbers, and export-controlled research data. If you have any questions about approved data types in Qualtrics, please contact Casey Hoerig (hoerig.4@osu.edu).

As a condition of maintaining your Qualtrics access through the College of Pharmacy, we ask you to please refresh your Institutional Data Policy training annually.

Data Management

Please note that Qualtrics accounts made available through the university are governed by Ohio State’s records management policies. Surveys and data stored in Qualtrics are subject to research data policies and university records retention schedules and practices, including requirements related to records destruction or transfer.

Survey Accessibility

When you begin to build surveys, please be aware that the university requires Qualtrics users to avoid using questions types that are not accessible. We strongly recommend that you always check survey accessibility and avoid using any of the identified inaccessible question types.
